Team Select Home Care - - Responsibilities: Provides services as ordered in the plan of care/aide care plan and that the aide is permitted to perform under State law; Performs/assists with direct patient/client personal care, household chores, meals, and other assigned/delegated tasks; Completes appropriate visit records in a timely manner as per Agency policy; Reports changes in the patient's condition timely as well as other needs to the RN; Attends staff meetings and patient care conferences as scheduled
