Responsibilities

• Provide comprehensive primary care under physician collaboration
• Assess, diagnose, and manage acute and chronic conditions
• Order/interpret diagnostics and create individualized care plans
• Coordinate care with specialists and document accurately in the EMR

Requirements

• Bilingual Spanish a plus
• Active NP licensure1-3+ years primary care experience preferred
• Strong communication, critical-thinking, and patient-relationship skills
• EMR proficiency and team-oriented mindset
