Schedule: Full-time; 4 - 10 hour shifts (Friday-Monday)

About the Role:

Hospice of Marion is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.

Key Responsibilities:

* Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.

* Provide individual and family counseling, crisis intervention, and bereavement support.

* Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.

* Assist with financial resources, Medicare/Medicaid, and long-term care planning.

* Maintain accurate electronic medical records (EMR) and participate in IDG meetings.

* Support patients and families with advance directives and memorial planning.

* Educate the community on hospice care.

* Participate in an on-call rotation.

Qualifications:

* Education/Experience: Bachelor's degree in social work or a related field; one year of clinical experience preferred.

Physical Demands:

* Ability to lift up to 25 pounds frequently and up to 250 pounds with equipment.

* Frequent mobility and possible exposure to communicable diseases and various environments.
