Coordinates case management activities for Medicaid Long Term Care/ Comprehensive Program enrollees. Conducts comprehensive evaluations of members using care management tools to ensure optimal healthcare outcomes. Develops proactive plans to address member needs, facilitating overall wellness. Ensures proper coordination with PCP and skilled providers for condition management and resource support. Utilizes negotiation and motivational interviewing skills to secure services necessary for member care. Empowers members to engage in their healthcare decisions and promotes healthy lifestyle changes. Critical Information Monday-Friday, 8:00 am-5:00 pm, with 50-75% travel for face-to-face visits in Miami Dade. Fully remote role. Job duration is ongoing. Education/ Licenses Needed Bachelor's degree required; social work degree or related field preferred. Case Management Certificate preferred.
