Chronius Health is the first virtual primary care practice built specifically for the 75 million people living with complex chronic conditions—patients with overlapping diagnoses, debilitating symptoms, and a long history of being misunderstood or underserved by the healthcare system. These conditions often include autoimmune diseases, fibromyalgia, long COVID, IBS, and other multisystem illnesses that fall outside the bounds of any one specialty. - Specialists often see complex patients whose care is fragmented, poorly coordinated, or dependent on them for more than just specialty expertise. Chronius steps in as the patient's primary care partner—managing day-to-day needs, coordinating across specialties, and ensuring continuity between specialist visits. This reduces unnecessary outreach, improves adherence to care plans, and allows specialists to stay focused on their area of expertise while knowing the patient has the support they need.
- Primary care providers are often overwhelmed by patients whose complex conditions require more time, coordination, and follow-up than traditional models can support. These cases strain schedules, increase administrative burden, and contribute to burnout. By referring to Chronius, PCPs can confidently transition these patients to a team equipped to manage their care more intensively—without compromising quality or continuity.
About the Role
We're hiring our first full-time team member focused on provider outreach and referral network development. This team member will reach out to outside providers to establish and develop mutually-beneficial relationships. This is a hands-on, ground-floor opportunity to build the systems, relationships, and materials that will help more patients access the care they need.
You'll work closely with our Head of Growth, care coordination team, and clinical leaders to create a robust flywheel for patient acquisition via a provider referral network. This includes:
Provider Outreach & Relationship Development
- Build and manage relationships with outside primary care and specialty providers—including cardiology, neurology, rheumatology, psychiatry, gastroenterology, and allied health professionals—across Florida
- Conduct in-person and virtual outreach to educate providers and office staff on Chronius' model, lead presentations, establish ongoing relationships, and serve as the primary point of contact for referral questions or feedback
- Collaborate with internal teams to develop and refine provider-facing materials and messaging that support strong engagement and clear communication
- Manage existing referral relationships on an ongoing basis to ensure continued referral flow
Referral Coordination & Operational Support
- Educate referring offices on the referral process and ensure a smooth patient handoff, onboarding, and continuity of care
- Partner with clinical and care coordination teams to streamline workflows and follow up on referred patients to confirm visit completion
Performance Tracking & Strategic Insights
- Track inbound referral activity volume resulting from your provider relationship development efforts, analyze trends, and report on referral volume, outreach effectiveness, and partnership development
- Share insights from the field to continuously improve outreach strategy, messaging, and operational processes
Who You Are
- 3+ years of experience in provider outreach-related roles such as provider referral network development, pharmaceutical or medical device sales, healthcare business development, care outreach, or partnerships within a clinical or health tech setting
- Proven ability to engage and build trust with a wide range of healthcare professionals—from front desk staff to CMOs
- Experience working directly with PCPs and specialists is preferred; strong understanding of primary care and chronic disease management is a plus
- Demonstrated success in growing referral pipelines, sales territories, or healthcare partnerships
- Familiarity with the Florida healthcare landscape and existing connections within provider communities are highly valued
- Exceptionally organized, self-directed, and results-driven, with excellent interpersonal and communication skills
- Energized by the opportunity to build from the ground up in a mission-driven environment focused on improving care for people with complex chronic illness
- Based in Florida with flexibility to travel locally up to weekly and regionally up to several times per month
- Bachelor's degree in healthcare administration, business, marketing, or a related field is preferred
Compensation & Structure

This is a full-time, contract-to-hire role with an hourly base pay of $22–$30/hour, plus a monthly performance bonus based on number of referred patients who complete their first visit. Bonuses are tiered and uncapped, with top performers earning well above target.
Total Year 1 compensation is expected to fall between $50,000–$75,000, with on-target earnings around $65,000/year. Travel reimbursement is provided for in-state provider outreach. After six months, there is an opportunity to convert to a salaried employee with benefits based on performance and fit.
